The Kátł’odeeche First Nation is seeking a
HOME SUPPORT WORKER (HSW)
Purpose
The Home Support Worker is a member of the health care team who provides personalized care for clients of all ages and their families in the client’s own home. They assist with activities of daily living (ADL) and household duties, ensuring safety and comfort while maintaining the client’s optimal health and independence. The Home Support Worker acts as a client advocate and facilitates communication between the clients, family, and other healthcare professionals to meet their physical, psycho-social, spiritual, cultural, and medical education needs.
The Home Support Worker reports to the KFN Director, Health, Social Services, and Wellness. The Home Support Worker works closely with the KFN Community Health Representative and the Hay River Health and Social Services Authority (HRHSSA) Home Care nurses.
The Home Support Worker is accountable for the types of services provided to clients and the scheduling of home visits.
Major Areas of Responsibility
The overall goal of the Home Support Worker is to assist elderly, ill, and disabled community members to remain at home in a safe and healthy environment for as long as possible by providing personal care and limited home-making services.
The main responsibilities of the Home Support Worker are:
• Providing personal care and hygiene services
• Providing home-making services
• Assisting with medical and other appointments and essential activities in the Town of Hay River, including transporting clients who do not have other means of travel.
• Maintaining client records; and
• Preparing reports.
